Diabetic kidney disease (DKD) is the leading cause of chronic (CKD) and end-stage (ESKD) in United States worldwide. Alterations glomerular hemodynamics, inflammation, fibrosis are primary mediators tissue damage, although relative contribution these mechanisms likely varies between individuals over course natural history diabetic disease. The presence DKD also strongly associated with cardiovascular morbidity/mortality has a major influence on survival. Clinical presentation prognosis heterogeneous vary individuals, severity albuminuria, particularly when combined elevated blood pressure, remains an important marker those at higher risk progression. Management requires holistic approach that combines reduction elements to slow progression disease, namely glycemic control, RAAS inhibition pressure lowering. Effective delivery interventions combination reduces risks progression, as well other microvascular complications, events, mortality. Several international groups have issued clinical guidelines largely agree recommended targets, practice should be tailored for each individual patient. SGLT2 inhibitors exciting new options now available nephropathy.
